District Volunteer Form

Make a Difference, Be a Volunteer!

Volunteers are an important component of the learning environment. The School District of Washington recognizes these efforts play a huge role in the success of our students. We encourage those who volunteer each year to continue, and others who wish to begin service to pursue the idea.


Due to board policy and the district’s efforts to keep every student safe, the district will require a Criminal Background Check on each potential volunteer every three years. A Criminal Background Check is a search of the Missouri State Highway Patrol’s criminal database and Sex Offender Registry, as well as the Central Registry of Child Abuse and Neglect of the Children’s Division of the Department of Social Services.


Please complete a volunteer form in full (one form per volunteer), and return it to your child’s school office at least two weeks in advance of any activity for which you would like to volunteer. The district must receive the results of the background check and officially approve the volunteer before he or she may begin service in the identified volunteer position. Please do not submit duplicate volunteer forms. We also suggest that you keep a copy of your dated volunteer form.


Upon receiving the results of each background check, approved volunteers will receive a 'volunteer badge' from your child’s school office. Please present this badge to school office personnel upon entering the building for volunteer service.


The district may decline the offer of any volunteer for any legal reason. All information collected on volunteers will be considered confidential to the extent allowed by law and will only be used to protect the students or minimize disruption to the educational environment.
